Postal Applications Berlin
There are two different forms used, depending on your circumstances:
Adults (18 and over) use the PC7 form (3 steps)
Children (under 15 years) use the PC8 form (6 steps)

ADULT RENEWAL - PC7 Form

APPLICATION FORM
Fill out the passport application at the global website (APO) and print it off (it is not possible to submit an application online). You will need to create an account first. When you fill out your application form online, you will automatically be directed to the correct form.
- Print: You may need to adjust your print settings so that the top/bottom of the page is not cut off.
- Sign: Make sure you signed within the white box.
- Date: Signatures are valid for six months only.
Please ensure that your email address is legible on the form and that it is an email address you use regularly.
After printing, make sure to come back here and follow steps 2&3.

PHOTOGRAPHS
We recommend that you have your photos taken by a professional photographer considering the photograph guidelines. Please be advised that photos accepted by other authorities are not automatically accepted for your Australian passport application.
- Ask the photographer for glossy photo paper.
- Ensure that the crown of your head is visible on the photo.
- Ensure that you are not wearing glasses on the photo.
- Write your name on the back of one photo and ensure that the ink does not transfer to the second photo.
- Do not attach the photos to the form.
Photos taken in a photo booth or at DM will likely be rejected as the photo quality does not meet APO standards. Selfies cannot be accepted.

- your application form,
- the two lose photos, and
- the completed credit card authorisation form
enclosed in an envelope (or else the security guards won't be able to accept it) here:

Once your new passport has been issued, it can be collected or it will be sent to you by registered mail (a postage fee applies).
Please indicate which option you prefer on the credit card authorisation form.

SIGNATURES
Page 84F: Parental Consent is required for a child passport. Everyone who is giving consent must sign section 15 of the form in front of a witness.
The witness:
- cannot be related to the child
- can but does not have to be the guarantor
- needs to provide their phone number below their signature
Page 85F: The lodging parent must provide a signature in section 17, date it, provide their full name and choose Parent 1 or Parent 2.
Children 10 years or older must provide a signature in section 18 unless there is a medical reason or disability that prevents them from signing. For children under 10, the lodging parent needs to tick the relevant box.

SUPPORTING DOCUMENTS
For mailed-in PC8 applications, the following supporting documents must be submitted:
- Copies of both parents' ID sowing their name, photo and signature
- Copies of both parents' proof of address (such as a residency permit, utility bill or Anmeldung).
Photocopies can be submitted in black/white or colour and must be of good quality.
Parent's name change: If the current name of either parent are different to how they appear on the child’s birth certificate, please include a certified copy of a document explaining the change of name (such as a marriage certificate). Certified copies for this purpose can be taken by the guarantor (step 5).

GUARANTOR
A guarantor must sign on page 83F of the application form and endorse the back of one photo using the wording "This is a true photo of (applicant's name)".
The guarantor cannot be related to the applicant, must either be Australian with a valid passport or a foreign national with a profession from the list (click on "Learn more" for a full list of guarantors).
If you have your guarantor to certify the copies required to prove a parent's name change, please ask them to write: "This is a true copy of the original document presented to me", with their full name, occupation or Australian passport number and the date signed.

If you live in one of the other states, please send your application to the Australian Consulate-General in Frankfurt.
